FORMS ARE DUE MAY 1ST
All families must complete the following 3 forms to attend ISTC. Campers with incomplete or missing items are not permitted to check-in.
- Physical Exam & Immunization Records (completed and signed by doctor)
- Health History Form (entirely online – found in parent portal)
- Attendance Agreement (entirely online – found in parent portal)
Once logged in, the space to upload or complete each item can be found under “Forms & Documents”.

PHYSICAL EXAM & IMMUNIZATION RECORD
For the 2026 season, the exam date for all physicals must be after June 1, 2025.
We have a template form you can bring to your doctor, although you can use any physical form that covers the same information. Many families send us physical exams they have used for school, sports, or other camps because the information overlaps.
All physical exams must be signed and dated by a doctor.
Participation eligibility forms do not meet the requirements of a physical examination form. We require the details of a recent physical exam visit with a doctor to ensure the best care for your child.
We understand that many insurance policies only cover 1 physical exam per calendar year. If your insurance prohibits you from getting a physical exam by the May 1st deadline, please email (office@international-sports.com) or text our office so we can work with you.
As an alternative, many urgent care facilities will perform physical examinations for a small fee.
Please upload your camper’s physical exam and immunizations in your parent account under “Forms & Documents” – there will be a space called “Camper Physical Examination Form”.
If your documents are in separate files, please upload the first one followed by the “Append” function for additional pages. Note, if you just click the upload button and not the append button, you will overwrite any previously uploaded documents.

COMPLETING ONLINE FORMS
The Health History is completed entirely within your parent account under “Forms & Documents”.
This online form will give you the space to outline your camper’s needs – medications, allergies, social needs, etc. It is imperative that you are as honest as possible, so that we can set our staff up for success in caring for your camper.
Any undisclosed medical condition that impacts your camper’s stay at camp may result in dismissal.
You will need your personal insurance card to complete the Health History. All campers must have medical insurance in order to attend ISTC.
Your personal medical insurance policy is your child’s primary coverage. All campers are covered by excess coverage accident insurance while at camp. If a camper sustains an injury at camp, parents are required to pay for any out-of-pocket expenses at the time of departure and then submit the claim to their insurance company.
Camp insurance does not cover illness. Parents must reimburse the camp for physician fee’s and prescriptions due to illness before departing.
You will need your camper’s medication information to complete the Health History. All medication and treatments, prescription or over-the-counter, must be logged on page 5 of the form.
All medication and treatments are given to the Health Center staff during check-in. You’ll have the opportunity to connect with our team in person during your check-in.
Please list any allergies and dietary restrictions in the Health History.
We can accommodate a variety of needs: gluten free, dairy free, vegan, vegetarian, nut free. We do not not use nuts, peanuts, or peanut oils in any of the food we prepare.

The Attendance Agreement is completed entirely within your parent account under “Forms & Documents”.
This form must be signed by a parent.
It outlines our expectations for behaviors at camp and its content should be reviewed with your camper.
